Information Requests & Privacy Policy

Ramsay Health Care is bound by the Australian Privacy Principles under the Privacy Act 1988 (Cth) and other relevant laws about how private health service providers handle personal information.

Requests for access to your medical records held by Joondalup Health Campus can be made by contacting the Privacy Officer.

If you require access to your medical records, you may apply by completing the attached application form. Two forms of identification (a photocopy of your driver's licence, passport, Medicare card, pension card, bank card etc. certified as a true copy of the original) must accompany the application form. Certification of identification for this purpose can be done at no charge by the Privacy Officer (or duly qualified person) at JHC when submitting this application. If you are requesting medical records/information on behalf of a patient, you will need to provide current authorisation in writing from that person allowing you to access their medical records. Two forms of identification from the applicant (a photocopy of your driver's licence, passport, Medicare card, pension card, bank card etc.) must accompany the application form.

Please note that the Privacy Act does not apply for deceased persons. If you are applying for medical records for a deceased patient, you will need to provide evidence that you are the Executor of the Will. If more than one executor has been nominated, all parties will need to sign the application form and provide certified ID as applicable. If the patient has not made a Will, the court may grant Letters of Administration to an appropriate person who may then make an application for a copy of the records. All documents provided in support of applications for deceased persons, must be certified by an appropriately qualified person (we are unable to provide this service at Joondalup Health Campus).

There may be an administrative charge involved in processing your request and providing access to the requested information. In the interests of providing you with a faster turnaround time and reducing the scope and cost of some applications, please consider if you actually require the entire medical record. If you are only after discharge summaries/certain dates/events, we would encourage/request that you apply only for the relevant information. Please nominate the dates on your application form or in your letter. This will greatly assist in processing your request more cost effectively and in a much shorter time frame.

You will be provided with an estimate of the administrative charge, which is to be paid prior to gaining access to the requested information.

Applications must be accompanied by two forms of ID (one photographic), that have been certified by a suitably qualified person. The application will not be processed without the certified ID.
